Design and Process FMEA: Rating, Prioritising and Reducing Risk

FMEA is often experienced as an endless table completed at the end of a project to satisfy a customer or an auditor. Ratings shift with the mood of the group, index values are adjusted to stay below a comfortable threshold, and the action plan never leaves the spreadsheet. Meanwhile, the failure mode that will trigger the next customer complaint has not been identified.

Two days of practice to run a design FMEA and then a process FMEA on real cases: breaking down the analysis, searching for failure modes, building rating scales suited to your organisation, prioritising and defining risk reduction actions. The work is carried out in a group, exactly as in a live working session.

1Framing the analysis

Knowing what the FMEA covers before starting

  • Position FMEA among the risk analysis methods used in design and production
  • Distinguish between the objects analysed in design FMEA and in process FMEA
  • Define the scope and break the system down into analysable elements
  • Gather the input data and assemble the working group

2Design FMEA

Working back from functions to failures

  • Describe the functions expected of the product or component
  • Identify the failure modes associated with each function
  • Trace back to causes and assess the effects for the user
  • List the detection means already in place at design stage
  • Complete the table as the session progresses

3Process FMEA

Following the manufacturing flow step by step

  • Break the process down into elementary operations
  • Identify the failures of each operation and their causes
  • Link effects to the characteristics of the finished product
  • Assess the detection means existing in production
  • Connect the table with the control plan

4Rating, prioritising and acting

Turning the table into decisions

  • Build severity, occurrence and detection scales suited to the organisation
  • Facilitate the rating round and handle disagreement within the group
  • Prioritise failure modes and set priorities
  • Define actions that address either the cause or the detection
  • Re-rate after action and close the analysis

Who is it for

Engineers and technicians from design, methods, production or quality functions who are expected to facilitate or take part in FMEA working groups.

Prerequisites

Experience of industrial design or production is required in order to benefit fully from the cases studied.

Practical details

Before the programme
Online positioning questionnaire. Your development objectives are shared with the trainer, who tailors the practical case studies to your context.
Teaching methods
Theoretical input, workshops and practical case studies. Digital course materials and method sheets provided.
Assessment
Multiple-choice tests and role-play exercises. Assessment of learning at the start and end of the programme, with immediate and 60-day follow-up evaluations.
After the programme
One year of access to the e-learning platform. Self-assessment of the skills acquired and a 30-day follow-up session with your trainer.
